Leber Hereditary Optic Neuropathy (LHON) is caused by mutations in mitochondrial DNA, which affect the energy production in cells, particularly in the optic nerve. These genetic mutations are inherited maternally, and symptoms usually appear in young adults, more commonly in males. Environmental factors such as smoking and alcohol use may increase the risk of symptom expression in genetically predisposed individuals.

Anterior Ischemic Optic Neuropathy (AION) is a condition where blood flow to the front portion of the optic nerve is reduced or blocked, leading to sudden vision loss. It is often linked to vascular risk factors such as diabetes, high blood pressure, or sleep apnea. AION can be either nonarteritic (NAION), the most common form, or arteritic (usually related to giant cell arteritis, a medical emergency requiring immediate treatment).

Ischemic Optic Neuropathy refers broadly to optic nerve damage caused by reduced or interrupted blood flow (ischemia). It can be classified as anterior or posterior depending on the affected part of the nerve. Ischemia deprives nerve tissues of oxygen and nutrients, leading to cell death and visual impairment. Prompt diagnosis is essential to manage risk factors and prevent further loss.

The experience of optic neuropathy can vary depending on the cause but commonly includes blurred vision, decreased color brightness, and dark spots in the central vision. Some patients describe a “gray” or “washed out” view, while others may feel eye pain (especially in cases of optic neuritis). Vision loss may be sudden or progressive and is often painless, except in inflammatory types.

Recovery depends on the underlying cause and how early treatment is started. In some cases, such as optic neuritis, partial or full vision recovery may occur. In others, especially where nerve damage is advanced, recovery may be limited.
