How Occupational Pain Develops
Work-related musculoskeletal disorders rarely begin with a single incident. They develop through one or more of three mechanisms and most occupational pain involves more than one operating simultaneously.
- Sustained static load
When a part of the body is held in a fixed position for extended periods, the muscles maintaining that position are under continuous low-level tension. This is distinct from the acute effort that produces immediate fatigue. It is more like holding a book at arm’s length for six hours, manageable in the short term, cumulative over time.
Office workers holding their neck in a fixed screen-facing position, dental hygienists maintaining lateral flexion over a patient, surgeons sustaining a precise posture during a procedure, all are experiencing sustained static load. The tissue tolerates it. Until it doesn’t.
- Repetitive movement under load
When the same movement is performed repeatedly, often under some degree of force or at the end of range, the structures involved — tendons, muscles, joint surfaces, nerves — experience cumulative microtrauma. Recovery between repetitions becomes insufficient over time.

- Psychosocial load and nervous system activation
